Our OPEN positions include:
Care Management Care Coordinator: The primary role of the Care Management Coordinator is to coordinate care for our members in the community setting and assist in addressing social determinants of health (SDoH) across our Care Management programs. The Care Management Coordinator gathers necessary information to develop an individualized care plan to address the member’s needs, health goals and care coordination. Requires outstanding communication, comfortable working in team environment and exceptional telephonic customer services skills. Full-time position Monday through Friday. No nights or weekends. Nurse Practitioner or Physician Assistant: This individual will coordinate care and make routine visits and clinical assessments in collaboration with the physicians, facility staff, patients, and families. The Nurse Practitioner or Physician Assistant will identify problems and treatment opportunities as early as possible, and provide education to individuals, their families, and other caregivers.
